Rehabilitation

Park Avenue Nursing & Rehabilitation Center is dedicated to providing quality outcome focused therapy services for all its residents. The short-term rehabilitation program is available seven (7) days per week and offers comprehensive rehab services. Long-term care residents receive therapy services to maintain their highest level of functioning.

Utilizing input from a team of medical, nursing, therapeutic, social service, dietary and recreational specialists, individualized care plans are developed with a focus on maximizing each person’s well-being and quality of life.

Outcome Analysis Study

During December 1, 2007 through November 30, 2008, 76% of the patients participating in the Short Term Rehabilitation Program either went home or reentered the community post-discharge from services. This is 6% better than regional statistics.*

All patients receive a comprehensive evaluation detailing their clinical and functional status at time of admission and discharge. A weekly goal tracker is utilized to inform caregivers, the patient and family members of progress made towards short and long-term goals.
